How to add a tab to a lightning app?
Click New in the Lightning Component Tabs related list. Select the Lightning component that you want to make available to users. Select the tab style and click Next. When prompted to add the tab to profiles, accept the default and click Save. Add your Lightning component to a Lightning app’s navigation.
Why is the Lightning tab not showing in Salesforce?

How to run Lightning component in your Org?
In order to run Lightning Components in your org, you will be required to use the My Domain feature. Follow these steps to create and activate my domain. Setup, enter My Domain in the Quick Find box, then select My Domain and then click Deploy to Users.
In New release we can’t customize the main Lightning Experience navigation menu.Since it can’t be customized, we can’t add Visualforce tabs,Custom Objects or apps to it. Can you add custom objects to lightning experience?
We can not use Lightning Experience until it allows for custom objects to be added to the navigation tab. This will be a major hurdle for user adoption. They have been spoiled with being able to customize their tabs which allows for quick access to the objects that are most important to them.
To open Visualforce tabs or Objects that are not added to the user interface elsewhere we need to use the App Launcher. @David, Agree with you. But they mention in documents like you can add Assets in Navigation menu. But we can’t add as of now.
