What is considered instrument time?
Instrument time is, as you say, when flight is conducted solely by reference to instruments. This can further be broken down into actual and simulated time. Actual time is when you are flying in cloud or visibility below VFR minimums and cannot navigate by visual references.
When can a Cfii log instrument time?
(g) Logging instrument time. (1) A person may log instrument time only for that flight time when the person operates the aircraft solely by reference to instruments under actual or simulated instrument flight conditions.
Can both pilots log instrument time?
Typically the same person does both. If the non-instrument rated pilot is manipulating the controls he can LOG PIC time, whether in IMC or under the hood. If not in IMC, the other pilot can either log SIC time, or if both pilots agree that the non-flying pilot is PIC, then they can BOTH log PIC time.

How long can you Fly an instrument flight?
The second way would be if you already have or had an instrument rating, probably a foreign one. In that case, the time logged under your foreign privileges is acceptable to the FAA for ratings purposes (see this question ). Note that your instructor will not sign you off for solo instrument flight after 15 hours.
