How are metadata fields stored in an application?

This needs to be considered with each insert statement written or executed at any stage.
Why are application metadata tools out of date?
A common problem with these tools is that they tend to be out-of-date and out-of-step with the actual application currently in operation. This is due to the fact that the definitions in them are not enforced in the operational programs or DBMS. They are passive repositories. If they are out of step, no one knows they are.
