What is parent record?
A one-to-one relationship (1:1) means that there is one child record for each individual parent record. Each student (existing in the parent form) would submit just one medical form at the beginning of enrollment (which would exist in the child form).
What is a child record?
A record lower in the hierarchical tree than a parent record; it is also directly liked to the parent and hierarchical databases.
What is a family day home?
Family day-homes provide care in the residence of the provider or the home of any of the children in care for up to 12 children under the age of 13 years, not including the provider’s own children and any children who reside in the home, when at least one child receives care for compensation.
Who is entitled to see a child’s school record?
Who Can See School Records. The privacy of school records is protected under state and federal law. The federal Family Educational Rights and Privacy Act (FERPA) gives parents or guardians (including anyone acting in a parental role in the parent’s absence, such as a foster parent or stepparent) the right to inspect their children’s records.
What should I do with my child’s medical records?
Determine how much information to release and to whom it may be released (parents, personal representatives, minors, emancipated minors, etc.). Develop policies and procedures for releasing medical records. Make sure your staff is trained on how to respond to complex cases involving minors. Get written authorization before releasing records.
Can a parent share a child’s medical records?

Can a stepmother get access to a child’s medical records?
A stepmother requests the medical records of her husband’s 9-year-old child. The woman doesn’t present any authorizations stating her authority to access the child’s records. Is the clinic obligated to give the stepmother access to the child’s medical records? No. One of the child’s parents would have to authorize the disclosure.
