What is rank of an element?
Rank of an integer in-stream is “Total number of elements less than or equal to x (not including x)”. If an element is not found in the stream or is smallest in stream, return -1.

How are all elements ranked in ascending order?
All elements are ranked from 1 to N in ascending order if they are distinct. If there are say x repeated elements of a particular value then each element should be assigned a rank equal to the arithmetic mean of x consecutive ranks.
