Why are images not showing on the form after upload?
I noticed that you are hosting images on your website from Google drive. I checked your form and I can see that Google drive is not allowing downloading of images. Google does not allow accessing files directly anymore. You have to view your file through their viewer.

Upload is not available in FormsPro at all. It is available for Forms regular but only for those that have are logged in on your domain. not for external users. so Sorry, no external users. Everyone wants this to change .. but it’s unlikely that Microsoft will open OneDrive (the area that files are uploaded to) to unidentified users.

Make the submit button the main image you are using. So the form tags would come first then submit button which is your only image so the image is your clickable image form. Then just make sure to put whatever you are passing before the submit button code.
